Quality of Life Task Force Asks Community to Plan Ahead

HYANNIS – The task force aiming to improve healthcare and the relationships between patients and physicians on Cape Cod wants residents to start planning ahead.

The Quality of Life Initiative visited residential complexes and community centers last week to encourage residents to fill out healthcare proxies and care directive forms.

A nationwide effort to encourage patients to communicate their wishes before a potential medical crisis spurred the local campaign.

Donald Guadagnoli, a senior medical officer at Cape Cod Healthcare, said he sees these cases often.

Educational materials and Health Proxy Forms are available at www.capecodhealth.org/qualityoflife.
