CCHC Seeks Community Benefits Grant Applicants

HYANNIS – Cape Cod Healthcare is seeking proposals from the community for its annual grant program.

The 2018 Community Benefits Grant program is a competitive process that is open to all non-profit organizations operating within Barnstable County with proposals that address chronic or infectious disease management or support initiatives targeting individuals or families facing mental health issues.

Specific chronic and infectious diseases of significance and growing concern in the county include cancer, cardiovascular disease, diabetes, Hepatitis C, HIV/AIDS and tick-borne illnesses.

The mental health issues of concern, which are identified by rates, counts, self-reported data and community input, include high or increased rates of depression, anxiety and suicide, use of hospital emergency departments for psychiatric care, declining availability of outpatient psychiatric care and access to available mental health services.

The 2018 grant program will provide up to $250,000 in grants to support programs ranging from $15,000 to $30,000.

The deadline for the submission of proposals is August 31.

Cape Cod Healthcare’s Community Benefits program promotes the organization’s mission to coordinate and deliver the highest quality and accessible health services in order to enhance the health of Cape residents and visitors.

In 2016, Cape Cod Healthcare invested more than $25 million in community health initiatives.
