Hospital Seeks Community Benefits Grant Applicants for 2018

HYANNIS – Cape Cod Healthcare is seeking proposals from the community for its annual grant program.

The 2018 Community Benefits Grant program is a competitive process that is open to all non-profit organizations operating within Barnstable County with proposals that address chronic or infectious disease management or improve the health of county residents and sustain the wellness of seniors.

Specific chronic and infectious diseases of significance and growing concern in the county include cancer, cardiovascular disease, Alzheimer’s and Dementia, Hepatitis C, HIV/AIDS and tick-borne illnesses.

Disease prevention and wellness priorities identified through self-reported health risk factors, health indicator data and community input in Barnstable County include health screenings, physical activities and nutrition, elder health and wellness and caregiver support.

The 2018 grant program will provide up to $250,000 in grants to support programs ranging from $15,000 to $30,000.
