Outer Cape Health Services Completes Provincetown Health Center Renovations

PROVINCETOWN – Outer Cape Health Services has announced the completion of the Patient Centered Medical Home renovation initiative at its Provincetown health center. 

Started in early 2017, the project received final sign-off approval from the Massachusetts Department of Health on July 14. 

Renovations to the Provincetown site were made to facilitate improved care coordination and communication between individual patients, and their care teams comprised of physicians, nurses and medical assistants. 

The Medical Home care delivery model is coordinated through a patient’s primary care physician to ensure patients receive the necessary care when and where they need it, in a manner they can understand.

“The Medical Home model of care puts patients at the forefront of care and builds better relationships between patients and their clinical care teams, “ according to Patricia Nadle, Chief Executive Officer. “Our goal is to deliver a patient-centered approach to care that results in happier and healthier patients. Research shows that the PCMH model improves care quality, the patient experience, staff satisfaction, while also reducing overall health care costs.”

Funded through a Health Resources and Services Administration grant and private donor funding, the renovation project comprised 2,100 square feet of the Provincetown health center footprint, including the following improvements:

– the location of four medical home “pods” to co-locate medical teams in close proximity of one another;

– the creation and equipment of three visitor care suites to increase access to urgent care within the medical home model;

– the construction of an acoustically screened consultation area in the on-site pharmacy to allow patients to consult with the pharmacist in greater privacy;

– the addition of a pharmacy call center office space for inbound and outbound calls to patients for increased convenience and specialized medication and compliance services.
